I started filling out the Form 1 for a Mag Light suppressor, and ran into a couple questions that could cause a delay. I was hoping someone who has a legal mag light suppressor could help me fill in the blanks.
4a. Origonal manufacturer. Im guessing, None. ?
4b. Silencer, not suppressor, correct?
4c. .30
4d. Model ... ?
Length
4e. of barrel. ...N/A. ?
4f. Overall. 7”

Do I need a responsible persons questionaire for an individual?
I have a trust, but it took too long for approval on the sbr. I will transfer to the trust in the future, but individual should be faster.

I Efiled my form 1 under a trust before the baloon went up and everything changed. Not a maglight but a form 1 supressor. The manufacturer was the trust. Yes I used Silencer. Mine was a 30 cal but it depends on what you are building it for. I'd use the "caliber of hole" it was intended for an not a chambering like .308. Model on mine was none. Length on mine was blank. Barrel on my form was blank. Can't help you one the second post but there is a lot of form 1 info on supressor talk and some how to's to be found with Google searches.

4a put your name and city state as it will be engraved on your suppressor (note the instructions show to put maker infor if creating a NFA item)

4c .30 is what I put
4d if you are going to engrave a model name on the suppressor then enter that name here otherwise put n/a.
4e I left that blank
4f yes put in your total length or even a little extra just incase

Description of Firearm and Markings. (1) Item 4a. If you are modifying an
existing firearm, enter the name and location of the original manufacturer.
If you are creating the firearm, enter the maker’s name, city and state.
(2) Item 4b. The types of NFA firearms are listed in the definitions; (3) Item
4c. Specify one caliber or gauge. If there is another designation, indicate
the designation in item 4h. (4) Item 4d. Show the model designation (if
known). (5) Item 4e and 4f. Specify one barrel length and overall length in
items 4e and 4f as applicable. Note: if the firearm has a folding or
collapsible stock, the overall measurement is to be made with the stock
extended. (6) Item 4g. Do not alter or modify the serial number of an
existing firearm . Enter the existing serial number or, if a new firearm, one
you create. (7) Markings: The maker is required to mark the firearm with
his or her name, city and state. All markings are to be in compliance with
27 CFR 478.92 and 479.102.

The following was several years ago:
I was a little surprised at how helpful ATF was with one of my applications. She actually called me to verify a change. It was one of those somantics type things where she could have just sent it back but called me and changed my form over the phone. They have a lot of my money. Maybe I got the good customer treetment .
It's been several years ago. They could have changed their attitude.

if your trust was approved, post 41F, you wont need to submit any of the extra paperwork - unless the responsible persons list has changed anyway.
fingerprints, passport photo's, etc - as i understand them - are valid for two years on file after approval of previous NFA item.
a quick call to the BATFE can/will be able to confirm that for you.
