Photos are ONLY necessary for a Form 23, and ONLY for the ATF (so 1 photograph per responsible person!). That's right... One photo per person. The form specifically states:
So, the ATF gets one copy, and the CLEO gets the other of the Form 23. ATF gets 2 copies of the Form 1 (you will get one copy back with the stamp), and your CLEO gets the third copy of the Form 1.Complete the form in duplicate. The ATF copy of the form, with fingerprints on Form FD-258 and photograph, will be submitted with the ATF Form 1, 4, or 5 (to the address
shown on the specific form) and the other copy will be directed to the responsible person's chief law enforcement officer.
As far as the fingerprint cards...
2 are submitted with each Form 23. None are submitted with the Form 1. The instructions on the Form 23 state:
