At the range today I had about 10 out of 30 rounds either double feed w/ the spent case and the next round, or the spent case get caught between the bolt and ejection port (pic below). When I put the can on for the next 30 rounds there was no issues. Also the brass ejected much further away as well.
I have a 8 inch Noveske barrel in a Viltor upper, standard buffer and spring. Using a 5.56 bolt out of a Stag upper. Firing Remington 115gr UMC

A buddy of mine had a similar problem. It was a worn out extractor for him. I'll try that out.

You should try an extractor O-ring, it looks like the extractor doesn't hold onto the rim.
