We suspected improper loading. When I disassembled the rest of the cartridges from the magazine, one of the cartridges had less than 2 grains of powder. One or more had loose bullets. Who really knows. Could have been the problem you mentioned.

Where would I find an Octane mount for this 45?

This happened to me after using some Barnes factory loaded black tips. The ammo was too hot and blew a primer into the gas key plugging it up. I had to drill the primer out. Pull the carrier out an try blowing through it. Any restriction means it's clogged up.
