I had this happen to me. It was the fault of the barrel in my case, not the suppressor. Another problem people have is using the compression ring that comes with a flash hidder on factory AR rifles. They may throw the can out of alignment.

I reluctantly called BATFE. They said I do need to fill out the forms (multiple because I cannot fit all of my stamped items on one form) but there was not a 'got to' by a certain date. Don't really have a recommendation other than stay away from aluminum. Aluminum in a can is really hard to clean without damaging the suppressor. I made the mistake of getting a lightweight, aluminum can for my .22 and it has been a nightmare cleaning it.
